# Form T6: Tenant Application about Maintenance

Ontario Landlord and Tenant Board.

Used for: A tenant's application asking the LTB to decide whether the landlord has not repaired or maintained the rental unit or residential complex, or has not complied with health, safety, housing or maintenance standards, and to order remedies.
