A party asks the LTB to assign another Member to complete a hearing or make a decision, because of the original Member's inability or delay.

• Choose mail or email on page 1 for how the LTB will communicate with you. The form says choosing email is consent to receive LTB documents by email.
• Sign the request on page 3.
