Form L2: Application to End a Tenancy and Evict a Tenant or Collect Money

A landlord's application to end a tenancy and evict a tenant after an N5, N6, N7, N8, N12 or N13 notice, abandonment or the end of a superintendent's employment, and/or to collect money the tenant owes (other than rent arrears).

Only the person can do

• Answer Schedule A Part B (compensation) on page 11 yourself: shade the statement about compensation, another unit, or the exception for an order to demolish or repair that is true for you.
• Answer Schedule B Part A (compensation) on page 13 yourself: shade the one statement that is true for you under the N12 reason you used.
• Choose how the LTB should communicate with you (mail or email) on page 4. The form says choosing email means you consent to receive documents and correspondence from the LTB by email.
• Complete the payment information yourself. Router.law never handles payment details.
• Sign the application on page 10 as the landlord or legal representative.
