## Only the person can do
- After choosing the benefit, the applicant or substitute decision maker signs Part 3, certifying the information is true and correct.
- The applicant chooses one benefit in Part 2 (income replacement, non-earner or caregiver). The choice cannot be changed after the form is submitted unless the injury is found catastrophic.
