Used for: Reports an assessor's estimate of the applicant's future attendant care needs after an automobile accident, for the accident benefits insurer.

- Assessor: complete Part 4 (weekly and monthly hours, the hourly rates that apply, monthly care benefits and the total). Router.law does not calculate them.
